Re-ablement Care


Re‑ablement is a short term support designed to help ensure you receive the right level of support at the right time to help you become as independent as possible. It may perhaps be that you require support with personal care or to prepare a meal. The re‑ablement service is designed to work with you to make it as easy as possible to manage these tasks on your own.

We aim to support you as you are discharged from hospital to reduce the time of your stay and can offer a more realistic assessment of your future needs.

With the re‑ablement service, a social worker/nurse will work with you so that you can re-learn important tasks needed for everyday life. Afterwards this can result in independence without reliance on social care support.

We operate a multidisciplinary team approach which comprises of our re-ablement care co-ordinator, social worker, district nurses, community matron, therapists, GP, your family related to your care programme and of course you.
